申もうし込こむとき、2人ふたりの関係かんけいを「世帯主せたいぬし」と「夫おっと（未届みとどけ）」と書かきました

When applying, I wrote the relationship between the two as ”head of household” and ”husband (undocumented).”

市しはこれを認みとめて、住民票じゅうみんひょうを出だしました

The city recognized this and issued a certificate of residence.
